This article explains how to use the root cause analysis (RCA) and corrective action plan (CAP) feature in the ClearStream module, and how to document and share your response to non-conformant wastewater test results.

01/ Introduction

The RCA/CAP feature works at the parameter level. Each failed parameter can have its own root cause and corrective action documented, and the combined output can be downloaded as a formatted PDF and shared with connected stakeholders. This provides significantly better insight and auditability.

The RCA/CAP workflow is on Step 7, after accepting results. Document root cause, corrective action, evidence, and resolution date for each non-conformant parameter.

RCA/CAP entries are saved at the report level — they are linked to the specific ClearStream testing cycle in which the failures occurred and remain accessible and editable for that cycle.

02/ How to open the RCA/CAP workflow

  1. In the ClearStream workflow, open the ClearStream & next steps step.
  2. Click Start RCA/CAP. The RCA/CAP page opens.

03/ Navigating the RCA/CAP page

The RCA/CAP page lists parameter groups from your test results in an expandable layout:

  • You can open any group to view sub-groups. Open sub-groups to see individual parameters, detected values, and compliance status.
  • Search or filter parameters by name, category, or status, such as filtering by Failed. Each parameter row shows the substance name, detected value, compliance status, and an RCA/CAP button in the action column.

04/ Adding an RCA/CAP entry

You can add an RCA/CAP entry for any parameter, but it is most relevant for parameters that have failed.

  1. On the RCA/CAP page, find the parameter you want to document.
  2. Click the RCA/CAP button in the action column. The RCA/CAP side panel opens for that parameter.
  3. Select a root cause from the dropdown (mandatory).
  4. Add optional notes on the root cause.
  5. Select corrective action from the dropdown (mandatory).
  6. Enter any additional details on the corrective action.
  7. Upload supporting evidence if available — attach a file to document what has been done or is planned.
  8. Enter the planned resolution date — the date by which you expect the issue to be resolved.
  9. If the issue is already resolved, tick Mark as resolved.
  10. Click Save.

A parameter with a saved entry shows an RCA/CAP tag in its row. You can also filter for parameters with RCA/CAP entries.

05/ Updating and resolving RCA/CAP entries

RCA/CAP entries can be updated at any time. Open the side panel for an entry, edit fields, and save.

Entries cannot be deleted. If an issue has been resolved, mark it as resolved using the Mark as resolved checkbox. The status updates from Pending to Resolved in the RCA/CAP report.

06/ Downloading the RCA/CAP report

After documenting failed parameters, you can download a PDF summary of all RCA/CAP entries. 

On the RCA/CAP page, click Download PDF in the top right. The PDF contains an overview of all RCA/CAP entries, including the root cause, corrective action, notes, planned resolution date, and current status (pending or resolved) for each parameter. This report can be shared with brands, auditors, or any other stakeholder who requires documentation of your corrective response.

07/ Tips & Notes

Tips: 

  • Use the Failed filter at the start of the RCA/CAP process to see only parameters that need attention. This avoids having to scroll through the full parameter list.
  • You can return to the RCA/CAP page at any time after the report is generated. Entries are not time-limited — you can continue updating them as your corrective actions progress.

Note: The root cause and corrective action dropdowns are mandatory. Entries can't be saved without both being completed.
